For the safety of all dogs in our care, we require proof of current vaccinations before any grooming appointment.
Please use the form below to submit your dog's vaccination records. We accept records directly from your veterinarian, printed copies, or photos of your vaccine card.
Required Vaccines
- Rabies — Every 1 or 3 years depending on vaccine type
- DHPP — Every 1 or 3 years for adult dogs
- Bordetella (kennel cough) — Every 6 to 12 months
